چکیده

Improved treatments for lymphatic filariasis (LF) could accelerate the global elimination program this disease. A triple drug combination of anti-filarial drugs ivermectin, diethylcarbamazine (DEC) and albendazole (IDA) has been shown to be safe effective achieving sustained clearance microfilariae (Mf) filarial parasite Wuchereria bancrofti from human blood. However, not previously evaluated treatment brugian filariasis, which accounts about 10% LF burden. This hospital-based clinical trial compared safety efficacy IDA with that standard (DEC plus albendazole, DA) in persons Brugia timori infections on Sumba island, Indonesia. Fifty-five asymptomatic B . Mf were treated either a single oral dose (28 subjects) or DEC (DA, 27 subjects). Participants actively monitored adverse events (AE) two days after by nurses physicians who masked regarding assignments. Passive monitoring was performed teams visited participant’s home villages an additional five days. Microfilaremia assessed membrane filtration 1 ml night blood at baseline, 24h one year treatment. more than DA completely clearing 24 hours (25/28, 89% vs. 8/27, 30%, P < 0.001). By 12 months treatment, only recipients had their (4%) 10 25 (40%) (P = 0.002). Approximately 90% participants antibodies recombinant antigen BmR1 baseline. Antibody prevalence decreased approximately 30% both groups months. About 45% experienced AE such as fever, muscle aches, lower back, joint abdominal pain. These mostly mild most common during first No participant severe serious AE. study showed well-tolerated significantly DA. Larger studies should further assess mass administration regimen eliminate filariasis. Trial Registration: NCT02899936

Efficacy, Safety, and Pharmacokinetics of Coadministered Diethylcarbamazine, Albendazole, and Ivermectin for Treatment of Bancroftian Filariasis.

BACKGROUND Available treatments for lymphatic filariasis (LF) are limited in their longterm clearance of microfilaria from the blood. The safety and efficacy of a single-dose triple-drug therapy of the antifilarial drugs diethylcarbamazine (DEC), ivermectin (IVM), and albendazole (ALB) for LF are unknown. METHODS We performed a pilot study to test the efficacy, safety, and pharmacokinetics of...

A randomized clinical trial comparing single- and multi-dose combination therapy with diethylcarbamazine and albendazole for treatment of bancroftian filariasis.

The Global Program for Elimination of Lymphatic Filariasis calls for mass drug administration for endemic populations outside of sub-Saharan Africa with a single dose of diethylcarbamazine (DEC) and albendazole (Alb) annually for 4-6 years. Single-dose DEC/Alb dramatically reduces blood microfilaria (MF) counts, but most treated subjects fail to completely clear MF after a single dose. A more e...

The Impact of Repeated Rounds of Mass Drug Administration with Diethylcarbamazine Plus Albendazole on Bancroftian Filariasis in Papua New Guinea

BACKGROUND This study employed various monitoring methods to assess the impact of repeated rounds of mass drug administration (MDA) on bancroftian filariasis in Papua New Guinea, which has the largest filariasis problem in the Pacific region. METHODOLOGY/PRINCIPAL FINDINGS Residents of rural villages near Madang were studied prior to and one year after each of three rounds of MDA with diethyl...

Comparison of Doxycycline, Minocycline, Doxycycline plus Albendazole and Albendazole Alone in Their Efficacy against Onchocerciasis in a Randomized, Open-Label, Pilot Trial

The search for new macrofilaricidal drugs against onchocerciasis that can be administered in shorter regimens than required for doxycycline (DOX, 200mg/d given for 4-6 weeks), identified minocycline (MIN) with superior efficacy to DOX. Further reduction in the treatment regimen may be achieved with co-administration with standard anti-filarial drugs. Therefore a randomized, open-label, pilot tr...

Effects of combined diethylcarbamazine and albendazole treatment of bancroftian filariasis on parasite uptake and development in Culex pipiens L.

We studied effects of combined diethylcarbamazine (DEC) and albendazole (ALB) treatment on Wuchereria bancrofti microfilaria (MF) uptake and development of infective larvae (L3) in Culex pipiens. Consenting Egyptian adults with microfilaremia (MF > 300/mL) were treated with one or seven daily doses of DEC/ALB. Laboratory-reared mosquitoes were fed on subjects before and after treatment. MF upta...
